How To Submit Concerns
- Submit an official Executive Directive Number 1 Concern email notification. Concerns must be submitted with DHRM within 180 days from the last incident.
- Complete all required fields in the email.
- Participate in process to resolve matters, which may involve personnel of your agency.
- Upon receiving your email notification, a system generated receipt confirmation will be sent.
