Missing Your ID Card
I am a state employee or non-Medicare eligible retiree group participant
To request a new COVA Care or COVA HDHP (High Deductible Health Plan) ID card:
- Step 1: To be mailed an ID card, your mailing address must be up to date in the Commonwealth of Virginia's system. Visit EmployeeDirect or contact your agency Benefits Administrator in the Human Resources Department to be sure that your mailing address is correct
- Step 2: If your mailing address is correct: Call the ID Card Order Line at 1-866-587-6713 . Provide either your health card ID number or your social security number. Two new ID cards will be sent to you within seven to 10 business days.
- Step 3: If your mailing address is incorrect: After taking the action in Step 1, wait 48 hours and follow Step 2.
To request a new COVA Connect ID card:
- Step 1: To be mailed an ID card, your mailing address must be up to date in the Commonwealth of Virginia's system. Visit EmployeeDirect or contact your agency Benefits Administrator in the Human Resources Department to be sure that your mailing address is correct.
- Step 2: If your mailing address is correct: Visit the COVA Connect Web site at www.optimahealth.com/cova , sign in to MyOptima, select “Request Member ID Card” from the menu, then follow the instructions. Or call Optima Health Member Services at (757) 687-6350 or toll-free at 1-866-846-COVA (2682),Monday – Friday, 8:00 a.m – 7:00 p.m. Once you place the order, your card should arrive in seven to 10 business days. Note: You may only request ID cards online once every 30 days. If there are less than 30 days between requests, call Optima Member Services.
- Step 3: If your mailing address is incorrect: After taking the action in Step 1, wait 48 hours and follow Step 2.
To request a Kaiser Permanente HMO ID card:
- Step 1: To be mailed an ID card, your mailing address must be up to date in the Commonwealth of Virginia's system. Visit EmployeeDirect or contact your agency Benefits Administrator in the Human Resources Department to be sure that your mailing address is correct.
- Step 2: If your mailing address is correct: Contact Kaiser directly at 1-800-777-7902 or (301) 468-6000. Your card(s) will be mailed to you, usually within seven to 10 business days.
- Step 3: If your mailing address is incorrect: After taking the action in Step 1, wait 48 hours and follow Step 2.
I am an Extended Coverage Participant
To request a new COVA Care or COVA HDHP (High Deductible Health Plan) ID card:
- Step 1: To be mailed an ID card, your mailing address must be up to date in the Commonwealth of Virginia's system. Visit EmployeeDirect or send your address in writing to the Office of Health Benefits, either by e-mail at ohb@dhrm.virginia.gov or by standard mail at 101 N.14th Street, 13th Floor, Richmond, VA 23219, to be sure that your mailing address is correct.
- Step 2: After taking the action in Step 1 , call the ID Card Order Line at 1-866-587-6713 . Provide either your health card ID number or your social security number. Two new ID cards will be mailed to your home address within seven to 10 business days.
- Step 3: If your mailing address is incorrect: After taking the action in Step 1 , wait 48 hours and follow Step 2.
